In the past few years there's an increased demand for gluteal augmentation or buttock implants. I do get a lot of requests to create a "JLO" behind. By far, my favored technique is fat augmentation, but some of my patients do not have enough fat that can be used for this purpose. In the massive weight loss patient a gluteal flap with fat grafts can be the solution to recreate an aesthetically pleasing buttock. Patients, who did not undergo weight loss and always have been "skinny", are limited to solid silicone buttock implants. Buttock implants are not at all like the implants used for breast augmentation. These are solid implants, come in different sizes and shapes, and are placed under the gluteus maximus muscle fascia or intramuscularly. These implants are placed via small incisions on either side of the gluteal crease, so they are inconspicuous when healed. They work best for upper or mid buttock augmentation to increase projection in these areas. They can be somewhat deficient inferiorly and laterally. If it is possible, I augment these areas with fat grafts at the same time.
Recovery is longer with implants than fat grafts, the implants tend to feel firm initially and there's more pain with the intramuscular implants. You may resume full activity in about six weeks. The implants might not feel fully a part of your body for as long as six months or longer.
